My Hospital Foundation (MHF) provides support to enhance the experience of patients, families, carers, volunteers and staff across the hospital and wider hospital community.
This form is to be used when requesting support from MHF for items, supplies, prizes, hampers, activities, events or other assistance that does not fall under the MHF Internal Funding Application (IFA) process.
Examples of requests may include:
Patient comfort items
Ward or patient activities
Event or activity supplies
Other small items or support that provide a benefit to
patients, families, carers or the hospital community
BBQ or food assistance
Arts and craft supplies
Toys and children's activities
Prizes or giveaways
Hampers
